چکیده

M e t h o d s Design: Randomized controlled trial. Allocation: Concealed.* Blinding: Unblinded.* Follow-up period: 90 days. Setting: Intensive care units (ICUs) in Geneva, Switzerland. Patients: 206 patients (mean age 63 y, 61% men) with acute renal failure (urine output < 200 mL/12 h despite treatment or blood urea nitrogen > 30 mmol/L with urine output < 1500 mL/12 h). Exclusion criteria were prerenal or postrenal failure, suspicion of glomerular disease, end-stage renal failure, or use of angiotensin-converting enzyme inhibitors. Intervention: CVVHDF (ultrafiltration flow rate 25 mL/kg per h replacement fluid plus dialysate flow rate 1 to 1.5 L/h for body weight < or > 70 kg, respectively) (n = 104) or CVVH (ultrafiltration flow rate 25 mL/kg per h replacement fluid) (n = 102). All patients received heparin (or periodic flushing with saline when heparin was contraindicated). Outcomes: Survival at 28 and 90 days, renal recovery, and duration of ICU stay. Patient follow-up: 100% (intention-to-treat analysis).

Comparison of Sustained Hemodiafiltration with Acetate-Free Dialysate and Continuous Venovenous Hemodiafiltration for the Treatment of Critically Ill Patients with Acute Kidney Injury

We conducted a prospective, randomized study to compare conventional continuous venovenous hemodiafiltration (CVVHDF) with sustained hemodiafiltration (SHDF) using an acetate-free dialysate. Fifty critically ill patients with acute kidney injury (AKI) who required renal replacement therapy were treated with either CVVHDF or SHDF. CVVDHF was performed using a conventional dialysate with an efflu...

Doripenem Dosing Recommendations for Critically Ill Patients Receiving Continuous Renal Replacement Therapy

Doripenem dosing regimens for patients receiving continuous venovenous hemofiltration (CVVH) and continuous venovenous hemodiafiltration (CVVHDF) were devised based on an established efficacy criterion (free plasma doripenem concentrations above the minimum inhibitory concentration [fT > MIC] of 1 mg/L for ≥35% of the dosing interval) while maintaining exposure below that with the highest studi...

Continuous Renal Replacement Therapy Applications on Extracorporeal Membrane Oxygenation Circuit

BACKGROUND AND AIMS Continuous venovenous hemofiltration or hemodiafiltration is used frequently in pediatric patients, but experience of continuous renal replacement therapy (CRRT) application on extracorporeal membrane oxygenation (ECMO) circuit is still limited. Among several methods used for applying CRRT on ECMO patients, we aim to share our experience on inclusion of a CRRT device in the ...

Renal replacement therapy in support of combat operations.

BACKGROUND Renal replacement therapy has been used by the U.S. Army at the combat support hospital echelon of care since the Korean conflict. Although there has been a general decline in the incidence of wartime acute kidney injury, the mortality associated with acute kidney injury and the use of renal replacement therapy remain unchanged, in the range of 60% to 80%. The U.S. Army official doct...

The clearance of protein-bound solutes by hemofiltration and hemodiafiltration.

BACKGROUND Hemofiltration in the form of continuous venovenous hemofiltration (CVVH) is increasingly used to treat acute renal failure. Compared to hemodialysis, hemofiltration provides high clearances for large solutes but its effect on protein-bound solutes has been largely ignored. METHODS Standard clinical systems were used to remove test solutes from a reservoir containing artificial pla...
